Applying for help from The Madeline Centre is easy. Anyone can refer their child and any agency can request help by filling out the Client Referral Form- Intensive In-Home or the Client Referral Form- Day Treatment online and submitting to us. You are also welcome to download the Client Referral Form (.doc ) , fill out and FAX it to us at (434) 239-0181.
The procedure for services depends upon the funding source. There are two main funding sources, Medicaid and FAPT or FAST. Medical insurance does not provide reimbursement for our services.
Medicaid
If a child is covered by Medicaid he/she would qualify for in-home services if the child is at risk of being placed out of the home or out of school. The person who makes the referral needs to only submit the Client Referral Form to The Madeline Centre and a person from our staff will contact you immediately.
FAPT/FAST
FAPT stands for Family Assessment Planning Teams and FAST stands for Family Assessment Services Team. These teams are funded by the state and are located in each county and city to provide for the needs of children… FAPT/FAST can often be a source of funding for children not covered by Medicaid. They also provide funding for services not covered by Medicaid such as behavioral specialists in the schools, mentors, and parent aides. These services are usually requested by a referring agency such as the school, social services or court services.
